- Please remember to make sure your doctor is billing correctly for preventative services. Preventative services are covered at 100%(no cost to you). Your EOB(Explanation of benefits) will detail what your doctor has billed for.
-Teledoc- This service works best if you pre-register. No one wants to register when they are sick! Sign up BEFORE you need to use this service. The cost is $45 per consultation, unless you have reached your deductible.
Eligibility For Insurance Benefits:
If you are a full-time employee (working a minimum of 4 hours per day), you are eligible to enroll in the benefits. The following family members are also eligible:

Your spouse unless legally separated

Your civil union partner
-
Your domestic partner
-
Your children according to age definitions
You have 30 days from your eligibility date to enroll in your benefits.
Life Events:
Make your elections carefully during your enrollment period because you can only make changes during the year if you have a Qualified Life Event (QLE). The following events qualify for a mid-year change in coverage:

Marriage

Birth or adoption of a child

Loss of other coverage

Change in your employment status or that of a spouse

Divorce or legal separation

Entitlement to Medicare/Medicaid

A qualified domestic relations order or a similar court order

Ineligibility of a dependent

Significant change in health coverage attributable to your employment or that of your spouse
You can make qualified changes to your benefits within 30 days after the event occurs.
