In the event that it is necessary for a parent/guardian to make changes to their student's original bus information, the form may be obtained here. BusRegistrationForm
Forms may also be obtained in the main office of the student's school. After completing the form with the changes please submit the form back to the main office. The office staff will see to it that the bus company receives those changes. The bus company will then contact the parent/guardian to inform them when the transportation changes will go into effect.
